Stephanie has spent her career focused on providing clinical support to patients, and providers within the primary care and specialty settings. Her training and professional appointments have provided her with a robust background in team-based care, quality improvement practices, alternative payment models and value-based care programs. She has experience working and leading across professional disciplines, and creating best practices for quality care initiatives and healthcare utilization.
Stephanie has held professionally evolving roles in healthcare and is currently the Director of Population Health Management at Community Care Physician. In this role she is responsible for the oversight of practice improvement strategies in support of the quadruple aim, as well as multiple payer initiatives and incentives. In addition, Stephanie assists practices in their efforts related to CMS’s Comprehensive Primary Care program, Patient Centered Medical Home recognition as well as other state and federal programs.
Stephanie earned her Doctor of Pharmacy degree from the Massachusetts College of Pharmacy and Health Sciences in Boston, MA and completed her post-graduate residency training at Harvard Vanguard Medical Associates. She was among the first clinical pharmacists to receive board certification in ambulatory care pharmacy practice. Stephanie also serves on the Board of Advisors for the American Cancer Society and the American Diabetes Association, as well as the diplomat for the Academy of Managed Care Pharmacy for the local student chapter. Outside of work, she enjoys spending time traveling, and enjoying outdoors activities with her family.
