WHO WE ARE
People have complex health-related social needs – whether it’s limited access to healthy foods, a safe place to sleep, or warm clothing – requiring streamlined coordination of care between a variety of providers. At Healthy Alliance, we lead the way in transforming how health care is delivered, shifting focus from singular medical interventions to a more proactive approach that addresses the whole needs of a person on a much larger scale.
By addressing health from all angles, we are cultivating the type of change that improves health and empowers the underserved.
WHAT WE DO
Our team, technology, and processes support successful connections between community members and our network of hundreds of local human services, behavioral, and clinical providers who offer a wide range of resources that are essential to one’s health and well-being.
The community member is always at the center of our work; our goal is to address community members’ health-related social needs – ranging from navigating and securing benefits, furnishing a home, dependable transportation, and so much more – before they turn into costly medical problems.
HOW IT WORKS
We’re an extension of our partners’ team, supporting both the evolving needs of their organization and the community members they serve. When a community member has a health-related social need, our local support team works to find and connect them, via our referral platform, to the appropriate service provider(s) that are ready and willing to help – both in and out of our network.
TOGETHER, WE ARE HEALTHY ALLIANCE
Our network brings together organizations, big and small, to coordinate and collaborate so that all communities have reliable access to the resources they need. Our mission of improving health is only made possible because of our partners, who show up every day for those who live, learn, work, and play within their communities.
We can help more people when we work together. Come see how joining our network can help both your organization and the community members you serve.

LOCAL RESOURCES TO MEET YOUR NEEDS
Our local team works to get you connected to resources that are nearby – whether it’s healthy food, help with doctor’s appointments, finding a counselor, getting enrolled in an educational program, and more – for free.
With ~1,300 partner locations spanning 25 counties in New York State, we believe everyone should have the same opportunity to be healthy.

Today was seriously SWEET! 🍬🍫
We satisfied a candy craving as we revealed the 2024 Q3 recipient of our Quarterly Employee Recognition Award at today’s all-staff meeting.
Starbursts because she’s an absolute star; Nerds because she’s full of insight and knowledge; 3 Musketeers because she’s team-oriented; and Wild Berry Skittles because she’s always ready to take wild times head on — all things that perfectly describe Sarah!
Congratulations, Sarah! We’re incredibly grateful for all you do! 🥳
While the candy will help fuel Sarah to keep communications on point, we couldn’t leave out the rest of the team from the sweet celebration. Shout out to Todd and Windy Hill Orchard for the delicious apple cider and cider donuts! 🍎🍩
