Capacity building is one of the core responsibilities of the Social Care Network (SCN) lead entity, which includes activities to further enhance the network’s ability to improve access to HRSN services. Per New York State guidelines, SCN lead entities may provide eligible CBOs with funding specifically to support capacity building for the CBO to more effectively deliver HRSN services.

WHAT IS IT:

The Momentum Fund 1 is for direct investments to support new, incremental, allowable expenses that include: New Positions, Purchasing Equipment, and/or Renting Office Space that are directly tied and fully dedicated to HRSN service provision (screening, navigation, enhanced care management, housing, nutrition, and transportation).

WHO IS ELIGIBLE FOR DOLLARS:

Not-for-profit (501 (c)(3) OR 501 (c)(4)) charitable organization that works at the local level to meet community needs and provide HRSN Services to Members, AND received an email from Healthy Alliance with letter attached informing that your organization has passed Healthy Alliance’s initial credentialing phase dated 3/31/2025 or earlier. Award payments are contingent upon Awardee executing a Healthy Alliance Participation Agreement and HRSN Service Provider Agreement with Healthy Alliance to provide HRSN Services to Medicaid Members.

FUNDS CAN

BE SPENT:

Dollars CAN be used for:

New Positions: Cost of new staff member once hired (salary plus fringe benefits) dedicated to providing HRSN contracted services.

Purchasing New Equipment: Purchasing equipment essential for and fully dedicated to HRSN service provision

Renting New/Additional Office Space: Renting new office space fully dedicated to the HRSN service provision the organization will be contracted to provide.

HERE’S AN EXAMPLE: A CBO is contracted with Healthy Alliance to provide pantry stocking for eligible Medicaid members, and requests capacity building funding for a new refrigerator to store fresh produce. The refrigerator is only used for the purpose of storing fresh produce for pantry stocking for eligible Medicaid members as part of the SCN program.

Dollars CANNOT be used for:

  • Existing expenses and positions
  • Capital expenditures/“bricks and mortar”
  • Office supplies and materials
  • Travel expenses (including mileage)
  • Costs currently funded by other local, state, and/or federal funding
  • Expenses currently being funded by capacity building dollars from another SCN
  • Partially dedicated equipment or space
  • Expenses for items already funded by Healthy Alliance for network partners

HERE’S AN EXAMPLE: A CBO is contracted with Healthy Alliance to provide pantry stocking for eligible Medicaid members. The CBO requests capacity building funding for a refrigerator to store fresh produce for the pantry stocking service, and also to hold fresh food and snacks for a locally funded community pantry program.

In general, “parent entities” may apply for capacity-building funds on behalf of a subsidiary that is a 501(c)(3) or 501(c)(4). The entity that receives and uses the funds must be a 501(c)(3) or 501(c)(4) and must use the funds toward delivering HRSN Services to Members.

Per New York State’s guidance, HRSN services in scope include screening, navigation, enhanced care management, housing, nutrition and transportation.

Yes. Organizations eligible to receive funding must be providing screening and/or navigation and/or enhanced HRSN services – does not need to be all.

HEALTHY ALLIANCE PARTICIPANT AGREEMENT (WITH BAA) ​
Agreement to become a Healthy Alliance Network Participant. Healthy Alliance is Healthy Alliance Foundation, Inc. and its affiliate Healthy Alliance Social Care IPA, LLC in this case. The purpose of the broader network is to represent all organizations that interact with a community member and identify the community member has a need and connect them to and/or provide services to address that need. The Agreement enables the provider to have access (license) to the Unite Us platform. There are no fees to join Healthy Alliance, and the participation agreement is non-exclusive. There are no fees to join Healthy Alliance, and the participation agreement is non-exclusive.

HEALTHY ALLIANCE HRSN SERVICE PROVIDER AGREEMENT (WITH BAA)
Agreement with Healthy Alliance Foundation Inc. This is the Agreement created by DOH for SCN Lead Entities to contract with HRSN Service Providers. This agreement only applies to providers who are contracted to provide the covered HRSN Services specifically – Screening, Navigation, Enhanced HRSN Services (Enhanced Care Management, Housing, Nutrition and Transportation) – to eligible members and who will be eligible to receive reimbursement for providing those covered HRSN Services. ​

CAPACITY BUILDING DIRECT FUNDING
Any direct capacity building funding awarded to a HRSN Service Provider is stipulated within an Exhibit to the HRSN Service Provider Agreement.

ADDENDUMS OR OTHER AGREEMENTS
In the instance a Healthy Alliance Network Participant is eligible for/chooses to participate in certain Healthy Alliance services (e.g. transportation) or a paid program, an Addendum to the SCN/IPA Participation Agreement or another Agreement specific to the service/program would be required. ​

Organizations that do not have a credentialed letter from Healthy Alliance as of 3/31/25 will not be eligible to receive funds from Momentum Fund 1. However, Momentum Fund 1 is one of many capacity building funds Healthy Alliance will release, so stay tuned for future opportunities to apply!

Organizations receiving capacity building funds from the SCN will need to provide an invoice and backup documentation to support the expenses to be paid. Once awarded, more information on documentation requirements will be provided.

CBOs must keep receipts, records and other financial documents related to the funding for the balance of each calendar year in which they were made and for six (6) additional years thereafter.

Eligible providers that provide services under multiple SCNs can apply for capacity building under both SCNs; however, the request for applied capacity building dollars under the SCN needs to be designated to the work of that specific SCN exclusively.

For time-based HRSN services, 30 days after receipt of approved invoices with required documentation for expenses incurred.

There is no maximum amount of funding that can be requested. Healthy Alliance will assess each request and determine the award amount based on the type and quantity of the request. In some cases, we may request additional feedback from the provider to gain a clearer understanding of their needs, and to ensure that the proposed request aligns with OHIP’s distribution guidelines and available resources.

Funding is not provided upfront. It is reimbursed based on the submission of appropriate documentation, which must be provided to receive the funds.
All funds will be distributed to the contracted provider once the expense has been incurred, the necessary documentation has been provided to Healthy Alliance, and the expense has been offset by the corresponding revenue that was billed.

We’ve allocated funding breakouts based on the DOH funding methodology for the SCN infrastructure funding and regional infrastructure needs.

Per State guidance, funds can be recouped. These terms and conditions will be outlined in the agreements between Healthy Alliance and HRSN service providers.

 

Correct, the award period is for 12 months. The goal is that funded positions would become sustainable after the 12 month period through HRSN service provision.

Yes, you would be requesting funds for a “partial” FTE. Please specify the % of FTE you are requesting – should be what % of that FTE’s time is dedicated to providing HRSN Services.
