THE MOMENTUM CAPACITY BUILDING FUND 1
DOLLARS TO HELP GET YOU READY

FREQUENTLY ASKED QUESTIONS
Do I need to fill out multiple RFAs if my organization covers multiple regions?
In general, “parent entities” may apply for capacity-building funds on behalf of a subsidiary that is a 501(c)(3) or 501(c)(4). The entity that receives and uses the funds must be a 501(c)(3) or 501(c)(4) and must use the funds toward delivering HRSN Services to Members.
What sort of health-related social need (HRSN) services are in scope?
Can an organization apply who is only planning on doing one or two of the HRSN services vs. all?
Yes. Organizations eligible to receive funding must be providing screening and/or navigation and/or enhanced HRSN services – does not need to be all.
Do you need to be a contracted partner to be awarded funds?
What’s the difference between the two types of contracts Healthy Alliance has for providers?
Agreement to become a Healthy Alliance Network Participant. Healthy Alliance is Healthy Alliance Foundation, Inc. and its affiliate Healthy Alliance Social Care IPA, LLC in this case. The purpose of the broader network is to represent all organizations that interact with a community member and identify the community member has a need and connect them to and/or provide services to address that need. The Agreement enables the provider to have access (license) to the Unite Us platform. There are no fees to join Healthy Alliance, and the participation agreement is non-exclusive. HEALTHY ALLIANCE HRSN SERVICE PROVIDER AGREEMENT (WITH BAA)
Agreement with Healthy Alliance Foundation Inc. This is the Agreement created by DOH for SCN Lead Entities to contract with HRSN Service Providers. This agreement only applies to providers who are contracted to provide the covered HRSN Services specifically – Screening, Navigation, Enhanced HRSN Services (Enhanced Care Management, Housing, Nutrition and Transportation) – to eligible members and who will be eligible to receive reimbursement for providing those covered HRSN Services.
CAPACITY BUILDING DIRECT FUNDING
Any direct capacity building funding awarded to a HRSN Service Provider is stipulated within an Exhibit to the HRSN Service Provider Agreement.
ADDENDUMS OR OTHER AGREEMENTS
In the instance a Healthy Alliance Network Participant is eligible for/chooses to participate in certain Healthy Alliance services (e.g. transportation) or a paid program, an Addendum to the SCN/IPA Participation Agreement or another Agreement specific to the service/program would be required.
If my organization is eligible for receiving an Implementation Fund award, am I still eligible to apply for Momentum Fund 1?
Yes.
What is the opportunity for accessing these funds if you do not have a signed agreement by 3/31?
What sort of documentation will be required if I receive capacity building funds?
Organizations receiving capacity building funds from the SCN will need to provide an invoice and backup documentation to support the expenses to be paid. Once awarded, more information on documentation requirements will be provided.
CBOs must keep receipts, records and other financial documents related to the funding for the balance of each calendar year in which they were made and for six (6) additional years thereafter.
What if our org is working with another SCN and receiving capacity funding from that SCN?
Eligible providers that provide services under multiple SCNs can apply for capacity building under both SCNs; however, the request for applied capacity building dollars under the SCN needs to be designated to the work of that specific SCN exclusively.
If awarded, how fast will I receive funds?
For time-based HRSN services, 30 days after receipt of approved invoices with required documentation for expenses incurred.
Is there a maximum amount of funding that can be requested?
There is no maximum amount of funding that can be requested. Healthy Alliance will assess each request and determine the award amount based on the type and quantity of the request. In some cases, we may request additional feedback from the provider to gain a clearer understanding of their needs, and to ensure that the proposed request aligns with OHIP’s distribution guidelines and available resources.
Am I required to match funds received?
No match is required.
Is it reimbursement based, or do you get the money up front then submit documentation after?
What happens to any “unused” funds?
How was the amount per region determined – was it based on geographic size/number of partners, etc.?
We’ve allocated funding breakouts based on the DOH funding methodology for the SCN infrastructure funding and regional infrastructure needs.
What if an organization does not use provided capacity-building dollars for intended use as specified by the SCN lead entity?
Per State guidance, funds can be recouped. These terms and conditions will be outlined in the agreements between Healthy Alliance and HRSN service providers.
If we receive Momentum Funds to pay for a position, is that only for one year?
Correct, the award period is for 12 months. The goal is that funded positions would become sustainable after the 12 month period through HRSN service provision.
If we expand a staff person's time working from part time to full time to cover HRSN services, can we submit an RFA for a part of their work time?
Yes, you would be requesting funds for a “partial” FTE. Please specify the % of FTE you are requesting – should be what % of that FTE’s time is dedicated to providing HRSN Services.
Will reimbursement occur at the end of the funding year or more often, say quarterly?
As you incur expenses, you can invoice.
