WHO YOU ARE

The Data Governance Analyst is responsible for the development, implementation, and management of Healthy Alliance data governance policies and procedures by ensuring the integrity, privacy, and security of organizational data across all systems.


WHAT YOU’LL DO

  • Develop, implement, and maintain data management governance policies, procedures, and standards across the organization, in collaboration with the Privacy Officer and the Information Data Governance Board (IDGB), ensuring alignment with business objectives. 
  • Create and maintain data dictionaries, metadata repositories, and data lineage documentation. 
  • Conduct regular data quality assessments and implement data cleansing initiatives to improve data integrity. 
  • Monitor compliance with data governance policies and regulatory requirements, including but not limited to HIPAA and SHIN-NY/Qualified Entity (QE) policies. 
  • Develop and deliver data governance training programs to promote awareness and adherence to best practices. 
  • Assist in the implementation and management of data governance tools and technologies. 
  • Participate in IDGB meetings and provide regular updates on data management governance initiatives. 
  • Support data-related projects by ensuring proper data governance controls are in place. 
  • Collaborate with IT and security teams to ensure data protection and privacy measures are implemented effectively. 
  • Analyze data usage patterns and recommend improvements to data management processes. 
  • Stay current with industry trends and regulations related to data governance and privacy to ensure Healthy Alliance’s compliance with all applicable laws and industry standards. 


WHAT YOU’LL NEED

Education 

  • Bachelor’s degree in information systems, computer science, or a related field. Equivalent combination of education and work experience in a related field may be considered in lieu of degree requirements.

Professional Work Experience

  • 3+ years of experience in data governance, data management, or a related role. 
  • Strong understanding of data governance principles, methodologies, and best practices. 

Skills, Knowledge, and Abilities

  • Familiarity with data privacy regulations such as GDPR, CCPA, HIPAA, SHIN-NY, and QE policies. 
  • Experience with data governance tools and technologies (e.g., Collibra, Informatica, IBM InfoSphere). 
  • Proficiency in SQL and data analysis tools. 
  • Strong analytical and problem-solving skills. 
  • Excellent communication and interpersonal skills with the ability to explain complex concepts to non-technical stakeholders. 
  • Experience in creating and maintaining documentation, including policies, procedures, and data dictionaries. 
  • Knowledge of data modeling and data architecture concepts. 
  • Ability to work independently and as part of a team in a fast-paced environment. 
  • Strong project management and organizational skills. 
  • Certifications such as CDMP (Certified Data Management Professional) or DGPC (Data Governance and Privacy Certification) are a plus.

The Data Engineer is responsible for ensuring that Healthy Alliance’s data is reliable, accessible, and effectively utilized by designing, building, and maintaining the infrastructure and data pipelines necessary for efficient data storage, transformation, accessibility, flow, and integration.


WHAT YOU’LL DO

  • Design and implement scalable data pipelines to integrate and process data from diverse sources. 
  • Develop and optimize data warehouses and data lakes, ensuring efficient data flow and accessibility across the organization. 
  • Implement data quality frameworks and automated data validation processes to ensure data integrity and reliability. 
  • Develop and maintain robust error management practices, including error detection, logging, and resolution to ensure data accuracy and minimize data loss. 
  • Manage file exchange processes, ensuring secure and efficient transfer of data files between internal and external systems, including handling various file formats and protocols. 
  • Contribute to the adoption of modern data engineering practices, including real-time data processing and stream processing.

WHAT YOU’LL NEED

Education

Professional Work Experience

  • Minimum of 6+ years of experience in data engineering with a track record of implementing effective data solutions. 
  • Experience in the health care or social care services industries is preferred. 

Skills, Knowledge, and Abilities

  • Strong proficiency in SQL and experience with advanced database management systems. 
  • Solid experience with big data technologies such as Apache Spark, Kafka, or Hadoop ecosystem tools. 
  • Proficiency in Python, Scala, or Java with experience in developing data processing applications.

    WHO YOU ARE

    The VP of Manged Care and Population Health Initiatives is a key member of Healthy Alliance’s leadership team responsible for the integration of value-based contracting as part of the overall sustainability plan in collaboration with managed care organizations, clinical, behavioral health and SUD providers, government, community-based organizations, and other strategic partners. This position will lead and drive managed care contracts and monitor their deliverables, as well as oversee population health efforts and clinical and social care integration to support the organization’s strategic direction as it relates to contracting and value-based performance. 

    This role is accountable for the organization’s quality program and population health initiatives/programs, leading the strategy and design and championing new performance incentive strategies that align with Healthy Alliance’s mission and goals in the value-based environment. 

    Reporting directly to the COO, the position will also closely collaborate with Healthy Alliance’s Medical Director. 

    WHAT YOU’LL DO

    • Work with the Executive Team to identify key market drivers and develop initiatives with the Managed Care Organizations (MCOs) to address clinical and social care trends and future business development efforts in population health. 
    • Be the key point of contact for all Managed Care Organizations (MCOs). 
    • Manage MCO Account Managers. 
    • Drive population health and contract management oversight, care delivery innovation, payor relations and management of key contract deliverables. 
    • Serve as one of the organization’s key spokespersons on value-based contracting, educating employees, partners, health and social care leaders, government officials, and other stakeholders on the expected changes in health care transformation.
    • Lead the social and clinical integration planning processes to evaluate the utilization, quality, total cost of care, satisfaction, and overall marketplace competitiveness.  
    • Partner with the CSO on key stakeholder initiatives, community planning, and convening forums and activities that support the needs of each region. 
    • Review quality, utilization, and cost performance for all areas of all SCN and VBP contracting and develop interventions and strategies with the Regional Directors to improve overall network performance. 
    • Maintain a strong working knowledge of social care and clinical initiatives, and national & state health policy and health care regulation. 
    • Oversee Healthy Alliance’s Quality Program across the organization and be the accountable leader that provides oversight and direction to Healthy Alliance’s Quality Program and Integration and Quality Committee (IQC). 
    • Monitor performance indicators to ensure established network quality standards according to the Quality Plan. 
    • Monitor community member, network partner, and MCO level satisfaction and, with the COO, make recommendations and changes for improvement. 
    • Review, align, and provide oversight to the approved Quality Plan and make recommendations to the internal team and IQC as indicated. 
    • Monitor and manage network performance standards, and where appropriate, use the quality review process to address performance opportunities and make recommendations as indicated. 
    • Lead the collaborative development and implementation of social care Evidence Based Guidelines and Protocols to achieve defined performance standards and/or targeted benchmarks. 
    • Explore and support key strategic alliances with clinical and social organizations to enhance an integrated delivery model of care. 
    • Create environment where clinical and social care partners work together to establish programs that improve the quality and efficiency of care.

    WHAT YOU’LL NEED

    Professional Work Experience

    • A minimum of ten years of progressive experience in a health care system, with a background in quality management, performance improvement, and/or care redesign. 
    • Knowledge and application of performance improvement techniques/methodology like: LEAN/SIX SIGMA, utilization management, and the principles of the clinical/social care redesign/transformation as examples. 
    • Demonstrated success working within a risk/shared savings model for defined populations. A deep understanding of bundled payments, direct-to-employer arrangements, and Medicaid risk models. 
    • Experience managing relationships with managed care/health plans. 
    • Experience working in partnership with multiple constituents throughout an organization. This includes achieving consensus through presentations, thought leadership, partnership, and relationship-building throughout the organization. 
    • Progressive experience as a senior-level leader that includes experience in clinical/social integration, performance improvement, and population health management. 
    • A leadership role with demonstrable success in an integrated delivery system that has successfully managed risk for example in an IPA, an ACO, a CIN, or in a managed care or clinical setting. 
    • Experience in successfully leading quality, and performance improvement across the care continuum. 
    • Proven ability to collaborate and build relationships across the health care ecosystem. 
    • Demonstrated understanding of general principles of health care management and the complexities of alternative payment models and value-based financing systems, both in commercial and government payor environments. 
    • Experience in an operations-related setting with health informatics, analytics, and program development.

    WHO YOU ARE

    The Network Development Director is responsible for the strategic growth of Healthy Alliance’s network to fill service gaps and support quality referrals. This position works closely with senior leadership, Operations, and other internal teams with the explicit goal of recruiting and onboarding partners that can meet the needs of the community members we serve.  

    Experience and knowledge of Healthy Alliance’s regional footprint as well as various sectors of the community — including social care providers, local government, behavioral health providers, education systems, community-based organizations, and clinical organizations — is essential to leading Network Development successfully.

    WHAT YOU’LL DO

    • Lead the Network Development team to work strategically with the Performance and Referral Coordination teams to identify and fill gaps in Healthy Alliance’s network. Mentor and develop the Network Development team through the support of organizational culture and key deliverables. 
    • Support use of available tools, including regional market analysis reports, out-of-network referrals, existing and new partner referrals process, networking opportunities, and operational reporting to support growth strategy.   
    • Implement agreed-upon strategies that result in network-wide expansion within Healthy Alliance’s geographic footprint.   
    • Present team results and escalate issues to leadership, including recommendations for solutions.   
    • Work collaboratively with internal teams and provide Network Development guidance and expertise to cross-functional workgroups.   
    • Speak at and represent Healthy Alliance at community forums, workgroups, and state and local engagements. 
    • Influence Network Development across all business regions in collaboration with internal staff and strategic partners.
    • Convene community forums in Healthy Alliance’s geographic areas that align with the company mission, vision, and goals.   
    • Partner with the Marketing and Communications team to develop communication materials, presentation decks, and other relevant promotional materials and social media placements. Inform Marketing and Communications of strategy and activities.

    We are looking for someone in the North Country Region which includes Clinton, Essex, Franklin, Fulton, Hamilton, Jefferson, Lewis, St. Lawrence, Warren, and Washington counties.

    WHAT YOU’LL DO

    • Provide leadership, direction, and oversight of the region’s overall network performance.
    • Manage the region’s contracts, programs, and services to ensure all SCN & HRSN Program requirements are met.

                                                                WHO YOU ARE

                                                                The Regional Director is responsible for the ongoing development, planning, coordination, oversight, and performance of Healthy Alliance’s Capital Region network in support of both the SCN & HRSN Program requirements and the organization’s broader strategic vision, goals, and mission. In this role, the Regional Director ensures that Healthy Alliance’s integrated network includes a diverse ecosystem of partners including community-based organizations (CBOs), grassroot organizations, managed care organizations (MCOs), government entities, behavioral health and substance use providers, clinical providers, and schools to enable HRSN screenings, care coordination, and the delivery of social care services to expected performance levels.

                                                                We are looking for someone in the Capital Region service area, which includes Albany, Columbia, Greene, Montgomery, Rensselaer, Saratoga, Schenectady, and Schoharie. 

                                                                WHAT YOU’LL DO

                                                                • Provide leadership, direction, and oversight of the region’s overall network performance.
                                                                • Manage the region’s contracts, programs, and services to ensure all SCN & HRSN Program requirements are met.
                                                                • Lead regional workgroups with the goal of educating and learning from a diverse set of leaders across the region who are deeply involved with the current health care delivery system/landscape, reporting out to Healthy Alliance’s SCN Council on lessons learned/best practice/milestone progression.
                                                                • Proactively monitor the overall health of the region to develop strategies and action plans to improve regional performance alongside other members of the leadership team and community workgroups, including but not limited to:
                                                                  • Performance and quality measures,
                                                                  • Contract deliverables,
                                                                  • Program and service deliverables,
                                                                  • Network composition and adequacy,
                                                                  • Budget, and
                                                                  • Payment and billing.
                                                                • Act as the accountable leader for the region that communicates directly to the Leadership Team and the SCN Council on network performance.
                                                                • Provide leadership and drive the key performance measures and deliverables of the region where they are responsible.
                                                                • Ensure regional quality standards are met in accordance with the organization’s quality program, the SCN & HRSN Program guidelines, and contract requirements.
                                                                • Work with other members of the Leadership Team to ensure accurate and timely reporting to NYS and other contracted entities.
                                                                • Work, in collaboration with other leaders, to ensure the region can adequately meet the demands for social care services, provide sufficient service choices to community members, and support a “no wrong door” environment for Medicaid members to access services. This includes ensuring sufficient network adequacy to meet the demands to screen community members and navigate them to services, as well as monitor capacity of social care service provisions for the region – collaborating on plans to address gaps in the network.
                                                                • Lead and manage the regional SCN Council governing body, ensuring SCN & HRSN Program governance requirements for the region are met.
                                                                • Serve as the organization’s key spokesperson for the region, in collaboration with other regional directors, to better educate leadership, employees, network partners, government officials, MCOs, and other stakeholders on the health care transformation efforts for the Capital Region.
                                                                • Collaborate with other Healthy Alliance Regional Directors, other SCN & HRSN Program Lead Entities across the state, the New York State Department of Health, and the Statewide Health Equity Regional Organization (SHERO) to influence and support best practice strategies in the provision of social care service delivery.
                                                                • Serve as the regional liaison, partnering with community leaders on key stakeholder initiatives, community planning, and convening forums and activities that support the unique needs of the region.
                                                                • Support activities that foster long-term sustainability efforts in the Capital Region, including the development of future value-based contracting.

                                                                          WHAT YOU’LL NEED

                                                                          Education 

                                                                          • Bachelor’s Degree required. Masters preferred.

                                                                          Professional Work Experience

                                                                          • A minimum of five years of progressive experience in a health or social care system, with a background in one or more of the following: program management, care coordination, performance improvement, and/or care redesign.
                                                                          • Experience working in partnership with multiple constituents throughout an organization. This includes achieving consensus through effective communication, convening, thought leadership, partnership, and relationship-building throughout the organization.
                                                                          • Progressive experience as a senior-level leader that includes experience in clinical/social integration, performance improvement, and population health management.
                                                                          • Must have demonstrated experience with data and its use in practice to inform decisions.
                                                                          • A leadership role with demonstrable success in an integrated care coordination delivery system.
                                                                          • Proven ability to collaborate, build relationships, engage and align diverse partners across multiple disparate ecosystems.
                                                                          • Demonstrated understanding of general principles of health care management and the complexities of alternative payment models and value-based financing systems, both in commercial and government payor environments.
                                                                          • Experience within managing relationships with NYS Department of Health, other government entities, and managed care/health plans.

                                                                                              The Performance Consultant (PC) role works directly with network partners to ensure successful implementation and execution of Healthy Alliance contracts. PCs are responsible for managing partners’ participation and performance, assessing needs, and providing strategic development of plans to align partner performance with successful implementation of contract deliverables.

                                                                                                        Education 

                                                                                                        • Bachelor’s degree required. Significant and relevant work experience may be accepted in lieu of educational experience. Degree in health, social services, or a related field preferred.

                                                                                                        Professional Work Experience

                                                                                                        • Consulting, account management, health or business administration, social work, human service, or nursing experience preferred.
                                                                                                        • Experience with clinical process improvement and performance improvement.
