People have complex health-related social needs – whether it’s limited access to healthy foods, a safe place to sleep, or warm clothing – requiring streamlined coordination of care between a variety of providers. Our local team works to get you connected to resources that are nearby (healthy food, help with doctor’s appointments, finding a counselor, getting enrolled in an educational program, and more) for free.
With ~1,300 partner locations spanning 25 counties in New York State, we believe everyone should have the same opportunity to be healthy.

Healthy Alliance was officially designated as one of just 58 Community Care Hubs in the Country by the Administration for Community Living (ACL).

New York State's 1115 Waiver Amendment draft called out Healthy Alliance as a model social care network (SCN) - previously referred to as social determinant of health network (SDHN).
Central New York Award
Awarded the RFI from CNYCC to manage the social care network in Central New York. Read more here.
North Country Award
Awarded the Mother Cabrini Grant to manage the social care network in the North Country.
Health Plan Contract | Fidelis Care
A three-pronged partnership between our IPA, IHANY, and Fidelis - providing services to Fidelis Care members, specifically those with at least one chronic health condition who have not had a primary care visit in more than six-months. Read more here.
Covid Collaboration | Smart Thermometers
During the onset of Covid, our team led a collaborative effort to secure and distribute approximately 7,500 smart thermometers to community based organizations (CBOs) and the community members they serve in the Capital District. Read more here.
Health Plan Contract | CDPHP
Focused on CDPHP Medicaid members in the Capital District, working to alleviate issues related to social isolation and transportation.
Our organizations partnered again in the summer of 2020 to identify members who were food insecure to create a tailored meal plan and receive other health services, such as routine screenings and immunizations. Read more here.

Our IPA is Established
Healthy Alliance's Independent Practice Association (IPA) is one of the first IPAs in the country focused explicitly on social drivers of health.
